About Ballarat Hospice Care Inc
In 1984 a public meeting was held, attended by those concerned to achieve better care for people with life threatening Illness. A community based Committee of Management was then formed to develop a palliative care service. Ballarat Hospice Care Inc is an independent organisation which has served the greater Ballarat area since April 1987.

Ballarat Hospice Care Inc
serves people in the local government areas of Ballarat, Golden Plains, Hepburn and Moorabool Shire.

Ballarat Hospice Care Inc provides:
Nursing Care
Care and support are available at home.
Emergency on call is available 24 hrs per day.
Frequency of visits will be negotiated between nursing staff and patient / family on an on-going basis.

Nursing care may be shared with the community nurse in your area.

Equipment Loan
A range of equipment is available for free loan. This includes wheelchairs, commodes, hospital beds and other equipment to assist in home care. All requests subject to availability of equipment at the time.

Support Service
Volunteer Support is available for both patient and family in its daily activities. Support is offered in the following areas:
Assistance with transport
Shopping / errands, etc.
Day sitting with the patient
Companionship for the care giver
Ongoing bereavement support
A Hospice Support Worker provides support by just being there.

Pastoral and Bereavement Care
Is available as desired by the patient and family. The bereavement follow up program offers support to the family for up to twelve months.

Inpatient Admission
Should inpatient care be required by a patient, discu
ss this with a Hospice worker who can help facilitate this.

Hospice and Palliative Care
Is a concept of care for people living with an incurable illness and their families. It offers hope by helping a person live a meaningful life through:
Relief of pain and other distressing symptoms
Provision of culturally sensitive holistic care recognising that any illness impacts on the whole person - physically, socially and spiritually.
Respect for the person's autonomy and choice provision of support for carers, family and friends, including bereavement support.

At present the service provided by
Ballarat Hospice Care Inc is free of charge.

Referral
Any person entering the end stage of a medically diagnosed life threatening illness or their family/carer, general practitioner, hospital or any health professional can request assessment by
Ballarat Hospice Care Inc Continuing medical support by a general practitioner is advisable.

Assessment
A person referred to Ballarat Hospice Care Inc will be assessed prior to acceptance.

Friends of Hospice:-
Friends of Hospice are a group of voluntary men and women of all ages formed in March 1990. This group works throughout the year to support the present total care of Hospice as a free service. For information contact the Hospice Office on (03) 53331118

Sustained community support is essential to maintain a fully functioning hospice program. Donations and bequests over $2.00 are tax deductible.
